ADF Loan Application Process

Complete a loan application.
You may download the application by completing the Church Profile Form below, or contact a Church Services representative toll-free at 1-888-878-3060 or 719-268-2250 (AK, HI, PR) to have one sent to you.
ADF may require a site visit to meet with your governing board and view the property.
You must have congregational approval (board approval if you're not a church) to apply for an ADF loan for a specific amount.
Complete the loan application and send a copy to both ADF and your district office for DEXCOM approval.
